This form is for companies (or sole traders) to provide the relevant information to Charles Sturt for approval prior to commencing work for the University.

If you're only updating some of your information with us, you are only required to complete those relevant fields, in addition to the required fields below.

Please note - the total size of all your attachments combined must be below 18MB.
If you need to supply additional documentation, please email it directly to us at contractor@csu.edu.au (making sure to keep each email under 18MB)
