Community University Partnerships Report Form

Successful Community-University Partnership (CUP) Grant recipients are required to complete the Project Report Form as outlined in the acceptance of grant process.

This form allows you to provide key information on your CUP Grant application success along with at least one photo and appropriate Image Release Form (individual or groups).

Your submitted Project Report Form will be emailed to the Community Engagement Team for their information and records.

Please attach at least one high resolution photo which Charles Sturt can use on its Community Portal (website). File upload limit 6MB

Please attach the appropriate signed Image Release Form (individual or groups), providing permission from all individuals in the photo for use of the photo by Charles Sturt University, including on our website. File upload limit 2MB
